- Athletic Trainer System (ATS) Profile
- Insurance Information and Insurance Card (front and back) uploaded to ATS
- Emergency Contact Information completed on ATS
- Electronic Forms completed on ATS
- Physical Forms uploaded to ATS
- Sickle Cell Results uploaded to ATS (if you were tested)
No student-athlete is permitted to participate in athletics without completing an ATS profile and submitting all medical paperwork.
