All medical paperwork is due on June 29 of each year to allow our staff to process your profile

What should be completed prior to June 29

• Athletic Trainer System (ATS) Profile
• Insurance Information and Insurance Card (front and back) uploaded to ATS
• Emergency Contact Information completed on ATS
• Electronic Forms completed on ATS
• Physical Forms uploaded to ATS
• Sickle Cell Results uploaded to ATS (if you were tested)

No student-athlete is permitted to participate in athletics without completing an ATS profile and submitting all medical paperwork.
