ALL student-athletes must complete or update the medical information in the database before the start of your pre-season camp (insurance updates, contact information, medical history). All information is encrypted and saved with your unique ID.

Once logged in, complete the information requested in
EACH TAB, including:

General Information,
Contacts,
Medical History,
Immunization,
Insurance, and
Athlete Forms. First-year student-athletes and transfers will need to complete their Immunizations, Medical History, and First-Year Participation Form under Athlete Forms. Returners will need to verify their General Information, Contacts, Insurance (if expired or changed), and Medical History (if there were any recent surgeries.)

ALL STUDENT-ATHLETES:
In addition to ATS information, ALL student-athletes must complete the following forms annually AND submit a photocopy of their PRIMARY INSURANCE CARD (both front and back):
   • Insurance Program Acknowledgement
   • Insurance Information
   • Medical History
   • HIPAA form

RETURNERS:
• ALL Returners must check in with the Athletic Training Staff prior to participating in ANY athletic event to make sure all files are up to date.
• If at ANY TIME your insurance information changes, you MUST bring in your new insurance card and update your information IMMEDIATELY with the athletic training staff
• If you were NOT CLEARED at the end of your season, you must be evaluated by the athletic training staff and/or team physician before participating in any athletic events. All student-athletes will be evaluated by the team physicians during their third and fifth years.
FIRST-YEAR STUDENT-ATHLETES:Â
In addition to the form listed above and insurance card submission, you must also complete the:
• Medical Information Form/Physical - must be completed and signed by an MD or DO. The physical must be complete on this form. No other documents will be accempted
• Sickle Cell Acknowledgement Form - results are to be submitted with medical records
Additional Notes:
Any student-athlete who is currently taking prescription medications for Attention Deficit Disorder, Anxiety or Depression MUST have a letter from the prescribing physician ON FILE in the athletic training room by completing the ADHD Examination Form. The medication normally prescribed for the above conditions can result in a positive NCAA drug test. The information is required to be on-file prior to any drug testing administration as athletic eligibility can be affected by a positive drug test.
All student-athletes are eligible for random drug testing by the NCAA. In order to protect yourself and remain eligible to participate in collegiate athletes, please read the list of NCAA Banned Substances. This list can also be accessed under the NCAA Drug Testing tab.
